Supporting documents

You will need to include a few supporting documents alongside your application. It will help if you have these on hand before you get started:

• Your Notice of Assessment from the latest tax year

• Functional Assessment completed by your medical doctor.

• Three months (current plus two previous months’) of your bank statements.

All applications have to be filled out in their entirety prior to submission.

If you’re working online, you can save an incomplete application and come back to it later.
